In this two-part episode i speak with Martin Bromiley about the tragic case of his then wife Elaine who In 2007, died due to a medical error. The Bromiley case will be familiar to many of of our NHS colleagues but the lessons from this case can teach us stark lessons about our own leadership and teamwork.
In this follow-up part two we will go deeper into the Human factors science of Martin's story and how we can take lessons in designing for safety, learning from success and failure, communication with team members, having a plan & taking control.
Martin is an airline pilot with an interest in human factors and formed the Clinical Human Factors Group in 2007.
